1. Who was there, when, and where
Date, time, who went, which doctor or which place.
2. What changed
New, stopped, or changed. Copy the exact words from the paper you were given.
3. What was done
What happened in the room or in the house.
4. What needs to happen next, and who is taking it
One name and one day on each line.
5. What needs attention
One or two things for the next person to watch. Keep it short.
